As Registered Nurses, we saw a recurring problem in primary care: independent practices desperately wanted to provide high-touch, proactive care for their chronic Medicare patients, but their in-house staff simply did not have the hours in the day to manage the strict time-tracking and monthly outreach required by CMS.

We founded Continuum Care Management to solve this operational bottleneck. We built a fully turnkey, "incident-to" clinical extension that allows independent practices to scale their Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs risk-free.

Jake Garcia, RN, BSN, CEN | Co-Founder & Chief Executive Officer Jake Garcia is a Pennsylvania-licensed Registered Nurse with a background in emergency medicine, critical care management, and patient triage. Recognizing the immense financial and operational strain placed on independent clinics and influx of patients slipping through the cracks, Jake co-founded Continuum Care Management to provide a seamless bridge between modern healthcare software and elite clinical labor. He oversees the daily monitoring protocols, EHR integrations, and strict CMS compliance standards for our partner practices.

Lisa A. Garcia, BSN, RN-BC, NE-BC | Co-Founder & Director of Clinical Operations With over 20 years of clinical and leadership experience in nursing, Lisa brings a wealth of patient-facing expertise to Continuum Care Management. Throughout her leadership career, Lisa has successfully led high-performing teams and driven measurable improvements in key healthcare metrics. Lisa is a board-certified Nurse Executive and Gerontological Nurse. She has presented at national and regional conferences on topics including fall prevention, delirium, and innovative approaches to patient safety. Having navigated complex patient cases in the acute care setting as well as homecare, she understands that true chronic care requires empathy, consistency, and sharp clinical judgment. She ensures that every patient interaction meets the highest standard of care, acting as a trusted clinical liaison between the patient and the physician.
